CA Healthy Kids Survey

The California Healthy Kids Survey is administered every other year to students in grades 5, 7, 9, and 11.  It is sponsored by the California Department of Education and gathers information about youth behaviors such as physical activity, nutritional habits, alcohol, tobacco and drug use, school safety, and developmental assets.  It is completely anonymous and the results are compiled district and statewide.  This survey helps GUSD monitor student behaviors and attitudes regarding health and assists us in the maintenance of our health programs.

If you do not want your 5th grade student to take the survey, you can mark that on the form that will be sent home in September.  If you do not want your 7th, 9th or 11th grade student to take the survey, you may opt out of the process by completing the form below and returning it to your student's school by October 14, 2019.
